SQL Refactor menu item remains in SSMS or Visual Studio after SQL Refactor has been uninstalled

After SQL Refactor has been uninstalled, its menu item may still be visible in SQL Server Management Studio or Visual Studio.
If this happens, it will be necessary to remove these manually.
How to fix
- From the MS SQL Server Management Studio main window, click on Tools, then Customize.
- Leaving this window open, click and drag SQL Refactor down the page onto an empty space in the left window.
- This should remove it from the menu bar.
